2. Type or select the required information in the form and click Next.
3. This will take you to a page where you can verify the information you have just entered.
4. Be sure that the information (particularly your name and date of birth) in the application must be the same as indicated in your NSO copy of birth certificate.
5. Make sure that the e-mail address you will provide in the application is active and correct.

NOTE: Any misrepresentations (or giving false/incorrect information) in your online application will automatically invalidate your admission in the University.
